SUMMARY REPORT

Finances: Self-financed, however Advanced Shoulder Arthroscopy Course on 27th Feb 2015 & Total Shoulder Joint Replacement Course at Antalya on 28th Feb and 1st Mar 2015 as part of my Fellowship Training is sponsored by Prof. Mustafa Karahan.

I am very grateful to all those because of whom (Especially to ESSKA and ISKSAA) I am able to meet and make a continuously growing relationship with an excellent and kind professional figure, Prof. Mustafa Karahan, Acibadem University Group of Hospitals, Istanbul, Turkey. İ would also like to thank Prof. Metin Türkmen, Chairman of Acibadem University, School of Medicine, Department of Orthopedics.

I thank my colleagues and seniors in the Department of Health and Family Welfare, Government of India and my family for relieving me and to look after my work and responsibilities in my absence.

Special thanks to Mr. Manish Gupta, Consular General at Istanbul for his support, guidance and assurance.
During these four weeks fellowship period my exposure to various aspects can be summarised as under;

  1. Observation of many advanced sports injury surgeries on major joints: I was rotated in different Hospitals of the Acibadem University also with Dr. Baris Kocaoglu, Dr. Umut Akgun. Picked up many tips for improvement in the arthroscopic surgeries.
    • Shoulder
      • Long head of Biceps tendon tear,
      • AC joint injury,
      • Rotator cuff tear- supraspinatus and subscapularis repair,
      • Repair technique in a case of global multidirectional instability. How to optimally make use of postero - inferior portal,
      • Special techniques in suture handling and knot tying.
    • Knee joint
      • Revision ACL reconstruction
      • Menisectomy and meniscal repair
      • Total Knee Replacement.
      • Primary ACL
        • Tips on how to harvest the hamstring graft fast and efficiently,
        • How to minimize errors in Tunnel placement especially on the femoral side,
        • How to increase the graft thickness by optimal utilization of the length,
      • At what knee angle you should fix the tibial side of the graft.
    • Ankle and foot
      • Management of distal tibiofibular syndesmotic joint injury,
      • Ankle joint arthroscopic surgery for Talar cartilage lesion,
      • Morton’s neuroma excision,
      • Achilles tendon repair and debridement.
    • Elbow
      • Arthroscopic release in case of old fracture dislocation.
    • Hip joint
      • Arthroscopic repair of the torn labrum and debridement of the FAI.
  2. Out - Patient Clinic
    • The long term follow up of operated patients and detailed deliberation in examination and management of the sports injury cases.
  3. Academic and Research Activities
    • Few dedicated session for research planning
    • Scientific article reviewing
    • A topic for multicentre trial for our future association, even on the Sundays also.
  4. Training Centre (CASE) Activities
    • I had chance to practice the arthroscopic skills on computer assisted simulator (VirtaMed), at Centre for Advance Simulation and Education (CASE), Acibadem University, where I enhanced my arthroscopic skills and knowledge by repeated practice. Burhanettin Ataş helped me in this regard.
    • Prof Alp Bayramoglu (Anatomy Professor) is kind and very supportive, made available fresh frozen cadaveric Knee and shoulder joints at the state of the art Anatomy Dissection Hall for a guided dissection of the knee joint where under the guidance of Prof Mustafa Karahan dissected the posterior and lateral aspect of the knee joint and a shoulder joint where we did a pilot study for safe placement of Anchor sutures in the superior part of the glenoid.
  5. Other Hospital visits
    • Fulya Acıbadem: I had an opportunity to visit and interact with Prof Sedat Tolga Aydoğdu at the state of the art FIFA accredited sports training centre at Fulya Acibadem.
    • Marmara University Sports Sciences Research Center: Marmara sports complex where the skill laboratory was set up in 2001 to train young surgeons in workshop especially designed to improve the skills for arthroscopic surgeries.
    • c. Marmara University Hospital: Professor made arrangements for me to have a ward round with his former resident Dr Selim Ergun at Marmara State University hospital, a government centre.
  6. Attendance to Courses with Turkish CME Credit
    • I attended the Advanced Shoulder Arthroscopy Course on 27th Feb 2015 at Centre for Advance Simulation and Education (CASE), Acibadem University, Istanbul, Turkey.
    • Total Shoulder Joint Replacement Course at Antalya on 28th Feb and 1st Mar 2015 which was very interactive and hands on cadaveric and saw bone experience as part of my Fellowship Training sponsored by Prof Mustafa Karahan. The registration cost, travel and boarding for these courses was born and incurred by Prof. Mustafa Karahan only. These CME and workshops provided me an additional opportunity to interact with orthopaedic surgeons from other parts of Turkey.

In general people I met in Turkey are very kind and warm hearted and Prof. Mustafa Karahan is an excellent host apart from being a multitasking, workaholic perfectionist who is always cheerful in spite of sleep deprived sometimes. I feel honoured and privileged to be hosted by Prof. Mustafa Karahan, he has taken care of me like his own younger brother and few a times gone out of the way to made my stay comfortable and left no stone unturned to made it a life time memorable event be it the trip to historically rich Istanbul site seeing or of multiple lavish dinners at world famous and beautiful Bosphorus facing restaurants.

Sometime I felt that I even encroached on the family time, thanks to Madam Michele, in true regard a better half, not only allowed me that liberty but also invited my family to stay at their house in the next trip to Istanbul.

Looking forward to transfer the benefit of this fellowship in patient care, scientific reviews, establishment of skill training centre and collaboration of multicentric clinical trials.
